Dear Parents of Gallatin Athletics,
Congratulations on being selected to participate in the 2010-2011 athletic year at Gallatin High School. We are very excited that your son/daughter is coming to campus and hope that they will be successful in all of their endeavors here. We want to help make this one of the most positive life changing experiences that your son/daughter will have.
My name is Andy Yarbrough. I am a Certified Athletic Trainer and hold a State of Tennessee Licensure to practice Athletic Training. My duties include the athletic healthcare administration at Gallatin High School by prevention, clinical evaluation and diagnosis, immediate care, treatment, rehabilitation and reconditioning of athletic injuries. I am in my forth year here as Certified Athletic Trainer at Gallatin High School. My job is made possible through Sumner Regional Health Systems and their Sports Medicine Program that covers other high schools in the area. Our team physicians are Brian E. Koch, MD of Tennessee Orthopaedic Alliance and Andrew Dill, MD of Sumner Medical Group. Both are seen frequently on the sidelines of Friday night football here in Gallatin. Also, Sumner Station is just off Vietnam Veterans Bypass/ HWY 386; take Exit 12 for any Diagnostic or Physical Therapy services. Should your son/daughter have an athletic injury or other health concern, we have the people in place for their optimal health care. I plan to go to great lengths to take care of your son/daughter as well as humanly possible.
In order to prepare for the upcoming year, I am requesting your participation in completing the following information. All student-athletes must have completed a Physical Packet or will not be allowed to participate in any athletic activities. This is to be filled out and signed by parent/guardians. The forms are designed to help physicians and other medical personnel evaluate, process and report the student-athletes medical clearance back to their school. It is also is accompanied by Sumner County Board of Education and Gallatin High School forms necessary for participation in interscholastic athletics.
